Summit Surgical uses an innovative online pre-admission and registration system to gather the information we need to give you the best care possible. This is part of a routine admission process that patients are required to fill out before most doctor visits and/or hospital admissions and includes demographics, contact information, allergies, medications, and other health related information. Doing this ahead of your appointment will save you valuable time and headache!

Plan on completing the health questionnaire at least 48 hrs. before your planned procedure or appointment.

You can sign in to our HIPAA compliant and secure site at any time that’s convenient for you. If you don’t have time to complete the online questionnaires, simply log off and return when you have time. The information you enter in our system is yours to keep and reuse, and is accessible to you through our patient portal.

Our aim is to simply reduce the back-and-forth communication between you, your doctor and our facility.

Summit's Patient Portal (iMARSLINK) is a new, free personal health information management tool that gives you convenient 24-hour access to your personal health records and information. It allows you to easily and seamlessly track and manage your health records from the privacy of your home. You can sign up in one of two ways:

Email Invitation From a the surgery center, or Direct signup by you on the iMarsLink website.
